Getting a placement
All referrals are made via the Older Adults team at Staffordshire County Health and Social Care Departments. Admissions are planned with the customer and their family, carers and associated professionals. Information obtained helps to form an initial care and support plan, which is regularly reviewed and amended. Customers will have their income assessed and those with capital above the government threshold may be required to make a contribution towards care. Please ask your social worker for more information on care funding.
